Irregular wide QRS complex tachycardia in a patient with pulmonary hypertension: What is the mechanism?
نویسندگان
چکیده
Introduction Pulmonary hypertension (PH) results in right ventricular (RV) hypertrophy and dilation that could lead to the development of arrhythmogenic substrates for cardiac arrhythmias, which are important predictors and contributors to morbidity and mortality in these patients. To date, the mechanism of ventricular arrhythmia in patients with PH are less well understood. Here, we reported a case presenting sustained and nonsustained ventricular arrhythmia owing to the increased automaticity of the right Purkinje arborization in a patient with severe chronic PH and the promising effectiveness of radiofrequency catheter ablation in elimination of the ventricular arrhythmia.

Wide QRS complex tachycardia in a patient with Wolff-Parkinson-White syndrome and cardiomyopathy: what is the mechanism?
A 32-year old woman with severe mitral regurgitation. congestive heart failure, and Wolff-ParkinsonWhite syndrome developed frequent, nonsustained episodes of a wide QRS complex tachycardia several days postpartum. Figure 1 shows an ECG with telemetry lead I and modified chest lead (MCL) recorded while the patient complained ot" palpitations. The same pattern of initiation and termination of th...
متن کاملAdenosine-Responsive wide QRS complex tachycardia: what is the mechanism?
A 45-year-old man presented with an acute anterior wall myocardial infarction and underwent successful percutaneous transluminal angioplasty of the left anterior descending coronary artery. Twenty-four hours later, a wide QRS tachycardia at a rate of 108 beats/min that had a QRS duration of 150 msec was noted by telemetric monitoring. There were no symptoms associated with the tachycardia. A 12...
متن کاملWide QRS Complex Tachycardia
A 71-year-old man with a history of previous myocardial infarction was referred to our hospital for recurrent symptoms of palpitation and syncope. During an episode of palpitations, a 12-lead ECG (Figure 1) was obtained in the emergency room. The ECG shows a wide-QRS complex tachycardia at a rate of 167 bpm. The P waves are positive in lead II (↑) before each QRS complex, and the PR interval is...
متن کاملWidening of the QRS complex during atrial pacing: what is the mechanism?
An electrophysiologic procedure was performed in a 50-year-old man with a 20-year history of paroxysmal tachycardia. During sinus rhythm, the atrial-His (AH) interval was 70 msec, the His-ventricular (HV) interval was 45 msec, and the QRS width was 80 msec. During atrial pacing at a cycle length of 290 msec, there was widening of the QRS complex (Fig. 1). The transition from a narrow to wide QR...
متن کاملArrhythmias in Patients With Pulmonary Hypertension; A Narrative Review of the Current Literature
Incidence and prognostic relevance of supraventricular arrhythmias were evaluated in nine studies. These studies investigated patients with different forms of pulmonary hypertension. Supraventricular arrhythmias were more often found in patients with group two pulmonary hypertension. Common findings in these patients were elevated right atrial pressure and diameters and reduced tricuspid annula...
